Please complete and submit the forms below via Google Forms. If you require a printed copy, please contact the ATC office at (859) 498-1103.
The Montgomery County ATC is not operated by your local school district (Bath, Menifee, or Montgomery County Schools), and we can not obtain the annual enrollment data that you may have already submitted to them. We apologize for this inconvenience and appreciate your cooperation in providing this information.
